We welcome new patients to our practice.
We receive limited funding from the PCT to provide NHS dental services and therefore we have to limit the number of people we see on the NHS or our annual budget will be used up before the end of each financial year. As such we are only able to accept new NHS patients who fall into the following categories:
- Anyone under 18 as well as under 19 and at full time education.
- Pregnant women or with a baby under 12 months
- Charge exempt patients – ie those receiving the following benefits:
- Income support
- Income related ESA
- Jobseekers Allowance
- Pension Credit Guarantee
- NHS Tax credit exemption certificate
For a full list of those who do not have to pay for dental charges please click here to go to the NHS website. Please tell the receptionist which benefits you are receiving when telephoning. Proof of exemption will be required at your first visit. If you don’t automatically qualify for free dental treatment you may still be eligible by filling in a form HC1 available at reception. You will have to submit the form and wait for the reply before proceeding.
All other new patients will be seen on a private basis. A basic check up starts from £35 excluding x-rays, following which a treatment plan and estimate will be provided.
In order to speed up registration please click the button below to download and print our new patient registration and medical history form and bring the completed form with you at your first visit. We look forward to seeing you.
